Welcome to the registration page for the Fall 2024 Art Wellness Program organized by
the Veterans Support
Council
and Connected Paint Digital, hosted at the Broadripple CoHatch. The intention of this Pilot Program is to
introduce Connected Paint Digital as
an art-based wellness tool to
reduce stress, anxiety
and increase positive self-well being among the participants. There will be a trained art wellness guide
providing instruction to ensure a smooth engagement. Service Members, Veterans and their
family/professional caregiver are eligible and there is no cost.

By registering for this program, I hereby acknowledge and agree to the following:
Assumption of Risk: I understand that participation in this program involves inherent risks, including
but not limited to physical exertion, use of art materials, and potential emotional responses. I
voluntarily assume all risks associated with my participation.
Release of Liability: I, for myself and on behalf of my heirs, executors, administrators, and assigns,
hereby release and forever discharge the Veterans Support Council, Connected Paint Digital, their
officers, employees, volunteers, and affiliates from any and all claims, demands, actions, or causes of
action arising out of or in connection with my participation in this program, including but not limited
to personal injury, property damage, or any other loss or damage.
Documentation: I grant permission for the Veterans Support Council and Connected Paint Digital to
use documentation of the activity for improvement and educational purposes.
